Practice writing SOAP notes.

Your rotation shouldn't be the first time you write a real one. Work through realistic clinical cases, write the note yourself, and see exactly what you missed โ€” section by section.

Your Objective section listed findings but skipped pertinent negatives. Documenting what's absent is how you show you considered the differential.
How it works

Three steps, real feedback.

No templates to fill in and no autocomplete โ€” you write the note the way you would for a real patient.

1

Choose a case

Realistic patients across 20 specialties, from routine visits to cases that shouldn't be missed.

2

Interview & write

Ask the patient questions, then write your own Subjective, Objective, Assessment, and Plan.

3

Get graded

Every section is scored against what a preceptor expects, with the items you missed and a stronger note to compare against.

What you get

Personalized to you

Everything is built around one loop: write, get corrected, then fix the specific thing you keep missing.

Section-by-section grading

Separate scores for S, O, A, and P โ€” not just one number.

Personalized Coach

Reminders based on your own repeated mistakes, not generic tips.

Adaptive study plan

A weekly plan that adjusts to your actual weak spots.

20 specialties

Family Medicine through Rheumatology, with more added regularly.

Exam mode

Timed practice that mirrors real clinical pressure.

Model note comparison

See a stronger version of the note you just wrote, side by side.

For faculty

See your cohort before clinicals do

Students enter a class code; you get a dashboard showing where the whole cohort stands, section by section. Free classroom pilots available for NP and PA programs.

FAQ

Questions people ask

Your first case is free with full feedback included. SOAPy Pro unlocks unlimited cases, all 20 specialties, personalized coaching, and the adaptive study plan.

No. SOAPy is an educational practice tool โ€” not a medical device or clinical decision support system. It exists to help you practice documentation, not to replace supervised clinical training.

Every case is fictional, and you should never enter real patient information. Our Privacy Policy spells out exactly what we collect and why.

Both. Download the iOS app from the App Store, or use SOAPy directly at soapyapp.com on any device โ€” your progress syncs to your account.

Grading adapts to your role โ€” MS1/2, MS3/4, NP student, PA student, resident, or practicing provider โ€” so feedback matches what's actually expected of you.

About

Why I built this

FS

Hey โ€” I'm Francisco. I work in an ER and I'm in NP school, so I've spent more nights than I'd like staring at a half-finished note, genuinely unsure whether what I'd written was good or just long.

Nobody really teaches this part. You learn the medicine, then someone hands you a chart and assumes the documentation will sort itself out. My feedback usually showed up days later, if at all โ€” and by then I'd already made the same mistake four more times without knowing it.

So I built the thing I kept wishing existed. Real cases, a note you actually have to write yourself, and specific feedback while it's still fresh in your head. No real patients, no waiting, and you can be as bad at it as you need to be at first.

If it makes your first rotation feel less like guessing, that's the whole point.
